Goal and Objectives
The goal of the Care Coordination e-Conference is to explore nursing roles in care coordination as a means to promote patient-centered care. The conference explores ways to improve care transitions, facilitate interprofessional collaboration, and improve quality and safety across the care continuum.
At the end of this e-conference, participants will be able to
- Summarize the roles that nurses play in care coordination and the patient populations they serve.
- Provide examples of how they can integrate key care coordination concepts into their current practice to improve patient care.
- Identify current and emerging trends that are impacting care coordination.
