Hodgkin Lymphoma Clinical Resource Area - Treatment

Hodgkin lymphoma is a cancer of lymph tissue found in the lymph nodes, spleen, liver, and bone marrow. The improvements that have been made in the treatment of this disease to improve patient outcomes have been unmatched in any other cancer in the past four decades. The newly diagnosed patient now has an overwhelming likelihood of being cured. In fact, since cure rates have increased to such an extent, the overriding treatment considerations often relate to long-term toxicity.

Treatment by Stage

The National Comprehensive Cancer Network (NCCN) version 1.2006 provide guidelines for the treatment by stage of this disease:

Classical Hodgkin's Disease/Lymphoma

Stage I-IIA

Nonbulky Disease
Chemotherapy-chemotherapy + involved field irradiation (20-30 GY)

Bulky Disease
Combined modality treatment-begin with chemotherapy with involved field radiation (30-36 Gy)

Stage I-IIB

Nonbulky Disease
Combined chemotherapy + RT to the involved nodal regions

Bulky Disease
Chemotherapy + involved field RT

(For early stage disease the NCCN committee recommends ABVD for 4-6 cycles or Stanford V for 3 cycles)

Stage IIIA, IIIB, IV

Nonbulky Disease
Chemotherapy alone (ABVD) or Chemotherapy (Stanford V)/RT

Bulky Disease
Chemotherapy ± RT to bulky sites
