The image of the Washburn Setup in Figure 7 of Safe Handling of Hazardous Drugs (2nd ed.) (p. 41) is incorrect. The Foley catheter shown in the print and electronic editions of the book is a three-way catheter that could potentially lead to a leak of hazardous drug. The corrected figure can be viewed in the "Click to Preview" option. Electronic editions purchased starting in September 2014 contain the corrected image. We apologize for the error.
